DES Benefits Navigation

Arizona DES

Our case managers screen for and enroll eligible clients in SNAP, TANF, LIHEAP, and child care assistance through the Arizona Department of Economic Security.

Home Plus AZ

Arizona's most-used DPA program provides up to 5% of the loan amount for down payment and closing costs. PRC counselors guide clients from eligibility through closing.

Call, email, or complete our online intake form. A care navigator will respond within two business days to schedule your initial consultation.

Whole-Person Assessment

Your navigator conducts a multi-domain needs assessment covering housing, finances, employment, health, and family goals.

Individualized Service Plan

Together, you and your navigator build a plan with clear goals, timelines, and the specific PRC programs and community resources that fit your situation.

04

Ongoing Support

Your navigator stays with you — monthly check-ins, plan adjustments, warm handoffs between programs — until your goals are achieved and you're stable.
